Do you use the default ini file?

Did you try reading the tuning guide included in the zip file?

Run comskip with the debugwindow switch (-w) to see how it processes the recording.

example

comskip -w "c:\your recordings\this recording.mpg"

Check if it finds the black frames and the silences.

OK,

Follow this procedure
http://mk.kaashoek.com/comskip/viewtopic.php?t=34
Use a full length recording of at least 30 minutes containing at least one commercial break.
Preferrably a 60 minutes recording.

Comskip can not detect commercials in short (10 minutes or less) recordings.

You have to make the .ref yourselve.

run comskip with the .txt file as its only parameter.
comskip "c:\my dir\my file.txt"

then you do a visual edit (Use the F1 key for help)
write the edit out with the W key.
Then, rename the .txt file to .ref

The .ref is where YOU want comskip to fin the commercials.
I need it to see if comskip is doing a good job as I don't have the original MPEG

Some questions.

Do you record at a low bitrate? (lower then 4 MBits/s)
Do you have a noisy signal?

The bitrate is related to the quality level you use for recording.
If you record at low bitrate (or low quality) comskip will see a lot of "noise" making detection very difficult
